I installed xp and Vista before and didn't have this problem. No internet, No CD/DVD, No USB driver. Using OS X.7 or later make a bootable USB thumb drive before running Installer! Working... check over here

I find "boot camp" to be used and over used to refer to too many things. I think I’ve tried just about all the suggested fixes listed previously: USB2.0, USB drive in left hand port (although not sure if this was relevant to the iMAC) and the that freakin intelxhci laying cutely in that folder. you have to let bootcamp restart your computer (you can't close bootcamp and then restart from the menu yourself...you must let bootcamp do the restart) your usb drive shouldn't come out
